Everyday distance, screen and reading tasks considered when choosing a monofocal IOL

One focus means choosing where that focus goes

A monofocal lens gives you one main focus. A common plan is to target distance. If the eye lands close to that target, road signs and television may be clear without glasses. A phone, book, or small print will usually need help.

That is not a failure. It is the plan.

Imagine three objects: a road sign, your dashboard, and a phone in your hand. A distance-targeted monofocal is built mainly for the first one. The dashboard may be usable, but it is less predictable. The phone normally needs reading glasses.

The target can be moved closer. Or the two eyes can receive different targets—often called monovision. But moving the focus does not create extra focus. It changes which distance is clear without glasses.

Toric is a separate decision. It corrects regular corneal astigmatism; it does not turn a monofocal into a wider-range lens.

What the simpler optical strategy gives—and does not give

With a monofocal, you usually accept more glasses in exchange for a simpler optical strategy and fewer design-related night-vision compromises than many wider-range lenses. Computer glasses may be useful as well as reading glasses; “glasses only for tiny print” is not a safe promise.

But “monofocal” is not a promise of perfect night vision. Dry eye, corneal irregularity, residual astigmatism, the retina, the optic nerve, the pupil, and the final refraction still matter. Nor are all monofocal models identical.

Questions for your surgeon

  • What exact distance are you targeting in each eye?
  • At my real phone, monitor, dashboard, and driving distances, what should I expect without glasses?
  • Is my astigmatism regular and large enough for a toric model?
  • Were my measurements repeated after treating dry eye?
  • Would monovision fit my work and depth-perception needs, and can we simulate it first?
  • If I miss the refractive target, what are the realistic next steps?
